Who We Are
Unity Care is a COA-accredited 501(c)3 organization with a 32-year track record of helping over 25,000 young adults from foster care avoid homelessness and build economic mobility as they transition from foster care into adulthood. Annually, we serve over 500 young adults throughout the San Francisco Bay Area, Monterey, and Sacramento regions.

What We Do
Unity Care prevents homelessness among young adults aging out of care between the ages of 18-24 through a Housing-first Continuum of Care that consists of Housing, Education, Employment, Well-being, and Unconditional Care.
With our support, our young people re-engage in - and graduate from high school, enroll in post-secondary education, secure their first job, progress to meaningful career employment, access critical mental health services, and become financially literate. Combined, these services enable our young adults who have faced significant challenges in their lives to finally thrive.

How We Work
Too often, aging out of foster care leads to homelessness and adverse life outcomes. For 32 years, our Five Pillars of Success Program Model has served as our guiding light and the model responsible for positive outcomes of more than 25,000 young adults between the ages of 18 to 24.

Whom We Serve
We focus on current and former foster youth ages 18-24.
Foster youth are among the most vulnerable members of our community. Removed from their families of origin due to abuse or neglect, or both, life as a foster youth and ward of the state brings different challenges.
We know the systems gaps they face as foster youth and the impact their personal journeys have on their mental health and life outcomes.
Unity Care helps these young adults focus on their strengths, build self-confidence, and finally thrive.
