Odyssey (Wraparound Services)
Odyssey is Unity Care’s Wraparound program. The goal of the program is to maintain emotionally and behaviorally challenged children with their family and communities.
Wraparound is a model of service that focuses on strengths-based planning to meet identified needs for a child. Plans are unique and relational, seeking to empower those individuals invested in the success of the family.
The Wraparound process provides community-based, comprehensive services in support of children and adolescents with serious emotional and/or behavioral disturbances so they can be reunited and/or remain with their families.
The Wraparound Family Team guides families in the process to develop an effective support network, increase their sense of competence, and acquire new skills for managing the special needs of their child.
Parent Family Partners (a role central to this family-centered model) are employed to serve as mentors and coaches for caregivers based upon their personal experience caring for a high needs youth.
Odyssey accepts referrals across Santa Clara County from the Department of Mental Health, Juvenile Probation, and Social Services.
