Transitional Housing Placement Program (THPP)

Unity Care’s Transitional Housing Placement Program (THPP) is a transitional housing placement opportunity for current foster and probation youth, ages 16 to 18. The goal of the program is to arm youth with the necessary life skills to live independently upon exiting the child welfare system.

 

The THPP model allows youth to live with a roommate in a safe and secure single- family dwelling in San Jose. In addition, youth receive regular support and supervision from our THPP staff, county social workers, and connects youth to ILP programs in Santa Clara County.

 

The home is staffed with a full-time on-site Case Manager, a Social Worker/Therapist providing individual and/or group sessions, and on-site Residential Counselors.

 

THPP services include:

  • Educational Guidance
  • Employment Counseling
  • Assistance in reaching Emancipation Goals
  • Transitional Independent Living Plan (TILP)
  • Case Plan Emancipation Readiness
