Outcomes

Unity Care’s mission is to provide quality youth and family programs for the purpose of creating healthier communities through lifelong partnerships. One way we accomplish our mission is by helping our clients self-determine and achieve reliable, sustainable and measurable goals. We also pay close attention to prescribed positive outcomes expected by Santa Clara County, other contracting agencies, and grant providers. Unity Care’s Board Program Committee oversees data related activities, subsequently measuring programmatic effectiveness, quality of service, service delivery outcomes and audit performance.

 

In addition, Unity Care is also implementing a new electronic health record system, AWARDS. The time commitment to design and implement AWARDS is substantial but, once in place, will standardize documentation, support consistency of care, and more efficiently produce outcomes reports needed by our various stakeholders.

In 2012, we have successfully

 

  • 15% increase individuals served through our Outpatient Mental Health Clinic
  • Exceeded youth served in our Wraparound program by 36%
  • Doubled Therapeutic Behavioral Services, thus meeting the needs of more foster youth and families in need of mental health services
  • Met over 90% of (CCL) required compliance in our group homes, thus ensuring high quality care for foster youth directly in our care.
  • Accomplished a 78% graduation rate for clients receiving mental health services.
