What We Do Welcome Center

Welcome Center

Unity Care’s Welcome Center is the initial point of contact for placement officers and for youth and families enrolling into the agency’s programs.  Our Intake Coordinator gives a brief introduction to Unity Care’s programs and services.  This orientation ensures that each party has a clear understanding of the agency’s services, as well as roles, expectations, rights for the youth and/or family and Unity Care.

 

Each program has its own individualized admissions process, however, the Welcome Center is responsible for the intricate admission process for all youth placed into our Residential Treatment Group Homes.

 

Admission and Referral Procedures:

  • Call the Intake Department at (408) 971-9822 for bed availability and initial referral screening.
  • For residential referrals to San Jose, fax the referral packet to the Intake Department at (408) 971-2255.
  • Referral packets should contain as much information about the youth, their family and current circumstances as possible, so we can make an accurate determination of their suitability for our residential program. It will also enable us to determine which of our programs will be most appropriate and offer the best outcome.
  • All packets are reviewed thoroughly for appropriateness of services and program fit.
  • A face-to-face interview with each youth is scheduled once suitability is determined. Phone interviews may be conducted for youth who are located far away or when transportation is an issue.
  • Upon determination of acceptance the placing agency is quickly notified.
  • Intake paperwork is faxed to the placing agency and needs to be completed and returned before admission.
  • Transportation is arranged and scheduled with the placing agency.
