Transitional Housing Program (THP)

Each year thousands of children nationwide leave foster care at age 18 or 19 without a formal connection to families, and with minimal independent living skills.

Teens in group and foster homes are often recovering from chemical addictions, and many come from homes with physical, sexual and substance abuse. Without family, financial or emotional support, these young adults lack basic education, have few marketable skills and have little experience living on their own.

At age 18, they become homeless and are not prepared to live independently.
Unity Care’s Transitional Housing Program (THP) offers affordable and stable housing to young adults ages 18-21, as well as a host of independent living services that foster self-sufficiency. Unity Care offers transitional housing service in Santa Clara and Monterey counties.

Unity Care’s Transitional Housing Program provides affordable, stable, transitional housing
for young adults while they are going to school, receiving job training, securing sustainable
employment, or learning and practicing skills for independent living.

A commitment from our THP participants, along with consistent community support, results in an ultimate team effort to develop a life of independence.

These young people are often in the state of being homeless without any family or financial support. THP gives these youth an opportunity to build a foundation and lead meaningful,
goal-oriented and successful lives.

THP provides an environment for our young people to become more self-sufficient and to develop a sense of worth. THP services are tailored to fit the individual circumstances of the young adult in order to provide the greatest possibility of success.

Youth receive the following opportunities in the Transitional Housing Program:

JOB READINESS: Includes vocational assessment, assistance with acquiring employment, developing interview skills, nurturing marketable skills, and learning professionalism.

EDUCATIONAL ASSISTANCE: Includes academic assessments, GED preparations, assistance
in college admissions including completion of college applications, tutorial assistance, assistance with accessing scholarships and financial aid, and helping youth develop good study habits.

FINANCIAL PLANNING ASSISTANCE: Includes budgeting, bill paying, and opening bank accounts.

INDEPENDENT LIVING SKILLS: Includes housekeeping, shopping, meal preparation and time-management skills.

SAVINGS PROGRAM: Assists in preparing for transition into permanent independent housing.

TREATMENT TEAM MEETINGS: Focus on developing a strengths-based transition plan for the youth.

COUNSELING: One-on-one and group emotional support and counseling on an as-needed basis.

CASE MANAGEMENT: The youths’ needs are identified and a transition plan is designed and implemented to address those needs.
