Transitional Housing Program Plus
Santa Clara and Orange County

Each year thousands of children nationwide leave foster care at age 18 or 19 without any formal connection to families and as a result they often become homeless. Without family, financial or emotional support, these young adults lack basic education, have few marketable skills and have little experience living on their own.

Unity Care’s Transitional Housing Program Plus (THP+) offers affordable and stable housing to young adults ages 18-24, while they focus on attending school, receiving job training, securing sustainable employment, or learning and practicing skills for independent living.

THP services are tailored to fit the individual circumstances of the young adult in order to provide the greatest possibility of success, which include:
