REFERRAL PROCEDURE

  • Call the Youth Services Department at (408) 971-9822 for bed availability and initial referral screening.
  • For residential referrals to San Jose, fax the referral packet to the Youth Services Department at (408) 971-2255
  • Referral packets should contain as much information about the youth, their family and current circumstances as possible, so we can make an accurate determination of their suitability for our residential program. It will also enable us to determine which of our programs will be most appropriate and offer the best outcome.
  • All packets are reviewed thoroughly for appropriateness of services and program fit.
  • A face-to-face interview with each youth is scheduled once suitability is determined. Phone interviews may be conducted for youth who are located far away or when transportation is an issue.
  • Upon determination of acceptance the placing agency is quickly notified.
  • Intake paperwork is faxed to the placing agency and needs to be completed and returned before admission.
  • Transportation is arranged and scheduled with the placing agency.
