Intake and Admissions We, at Unity Care Group, accept children (ages 6 to 12) and youth (ages 13 to 21) into our programs by referral only. For information about specific Unity Care Programs click the links below: Level 11 Residential Programs – We provide welcoming, safe and nurturing home environments. Unity Care’s residential facilities are located in vibrant middle-class communities with quiet streets and manicured lawns. Our outreach programs like the Youth After-Care Leadership Program, Independent Living Program, Reaching Independence through Secure Employment and the Youth Leadership Council, work in conjunction to continually empower our youth socially, mentally, and spiritually. Level 11 Dual-Diagnosis Treatment Program – Unity Care’s Dual-Diagnosis Treatment Program integrates the treatment of chemical dependency with mental health services. This comprehensive program addresses the needs of youth with multiple diagnoses, such as alcohol or drug dependence, depression, anxiety, bipolar disorder, PTSD, and/or compulsive behaviors. Level 14 Residential Programs- Unity Care’s short-term Emergency Crisis Shelter and Sub-acute Psychiatric Program are located on a beautiful two-acre property in southern Salinas. Surrounded by strawberry fields and graze land, this campus is an ideal place for our fragile clients to heal and be renewed. We offer a Day Treatment Program, Therapeutic Behavior Services and an on site school. Transitional Housing – Unity Care’s Transitional Homes are located in lively middle-class communities close to public transportations and community resources. This program is uniquely designed to meet needs of youth who have experienced the agony of helplessness and dependence in foster care and/or the juvenile justice system. We provide a safe and healthy home environment for homeless young adults, ages 18-21. Each youth experiences emotional tranquility and physical safety, structure and boundaries, and they learn how to create and maintain symbiotic relationships. Through love, support and intensive case management homeless young adults become thriving, contributing, and active members of our community. REFERRAL PROCEDURE
