Director Clinical Services–San Jose

Req: 024-012

OVERVIEW:  This position is responsible for overall service development and delivery of quality services within Unity Care mental health programs.  Assures service quality, develops collaborative teamwork among and across service areas and teams for greater efficiency and client satisfaction. Plans for the ongoing development of agency services and builds strong community partners to establish positive representation in the county.


QUALIFICATIONS:

  • At least 5-10 years experience developing and overseeing program development and management of clinical settings with adolescents.
  • Master’s Degree in a behavioral science or equivalent.
  • At least 5 years of full-time employment as a mental health professional or social worker in an agency serving children and/or adolescents.
  • Licensure as a mental health professional in the State of California.
  • Knowledge and experience with the regional systems of care, funding mechanisms, and external quality measures.
  • Proficient in using Microsoft Office applications.

RESPONSIBILITIES INCLUDE (but not limited to):

  • Adhere to state, county, and organizational policies, procedures, and guidelines to ensure contract compliance.
  • Supervise, plan, and evaluate budget, programs, staff and fiscal resources assigned to the mental health programs including compliance with required documentation to meet audit standards and timelines.

  • Network with the community stakeholders and partners in order to develop new improved ways of providing mental health services to better meet the needs of at-risk and disadvantaged youth.

IT & Facilities Manager

OVERVIEW:  The IT & Facilities Manager will provide strategic technology vision and leadership for UCG to develop and implement IT initiatives that improve cost effectiveness in a growing agency. In addition, the IT & Facilities Manager will direct the operation, evaluation and maintenance of all UCG buildings, residential group homes, and equipment and transportation fleet. Under the direction of the CFO, establishes the most cost-effective maintenance and service plans provided by outside contractors and vendors of all equipment and supplies.


QUALIFICATIONS:

  • Must have 5 or more years of direct supervision or management of IT operation.

    1.       Experience moving legacy systems to a unified platform

    2.       Experience with web applications

  • Must have 5 or more years of experience in managing a facilities department and/or property management experience with at least 2 years hands-on management for multiple property sites.
    1. Knowledge of safety and OSHA requirements. Knowledge of CCL requirements preferred

Clinical Compliance Specialist

Compliance/Quality Assurance Officer

OVERVIEW:  The Compliance/Quality Assurance Officer will oversee and maintain a comprehensive Compliance Program to ensure compliance of Unity Care Group (UCG) with applicable rules, regulations and statutory requirements. The Compliance Officer will work closely with UCG’s Board, CEO, Executive and Operational leadership team to disseminate information regarding laws and regulations, as well as act as a resource and subject matter expert regarding regulatory issues affecting the daily operations and development of new products and services.   The ideal candidate will have extensive experience in ensuring that the agency is proactive in complying with regulatory and operational changes/updates, conducting compliance and quality assurance program reviews & audits, electronic record keeping and ensuring that the UCG is adhering to its policies.

  • Manage and be responsible for oversight and quality assurance of UCG’s service contracts and processes to protect UCG’s funding assets and to ensure compliance with mandates and legal requirements
  • Monitor and implement compliance and quality assurance standards, evidence base best practices, and procedures that insure service compliance and quality assurance.
  • Monitor and conduct compliance and quality assurance audits of All Programs (Residential, Mental Health and Community base services) and administrative functions.
  • Work closely with UCG’s Executive and leadership teams to review compliance & quality assurance standards, updates and operational changes
  • Provide reports on the progress of implementation of compliance standards, corrections of deficiencies and any areas of concern, to the CEO & executive and leadership teams, on a regular basis
  • Keep abreast of changes in regulatory and third party billing guidelines, policies and objectives, and other rulings

QUALIFICATIONS:

  • BA/BS in Business Administration, Management,  Finance, Legal, or related field; Masters or Licensure in Social Welfare or related field is preferred.

Clinical Program Manager - Wraparound

REPORTS TO: DIRECTOR OF FAMILY SUPPORT SERVICES

Working within the vision, mission and philosophy of the agency and program, and under the supervision of the Director Wraparound programs, the Clinical Program Manager develops, maintains and manages the wraparound program, staff and service delivery. Ensures development of infrastructure and the provision of quality services by program staff, and provides overall leadership and day-to-day supervision. Works flexible hours that may include evenings and weekends as necessary.

  • Oversees the development and provision of quality, culturally relevant Wraparound services to up to 24 high-needs youth and their families, utilizing Wraparound services and best practices in both community and in-home settings.
  • Provides individual supervision, mentoring, training and coaching for Facilitators, Family Specialists and Parent Partners to provide high quality effective and efficient services
  • Oversees and reviews mental health assessments and treatment plans, and (Family support ) wraparound plans and, assists with child and family team meetings as needed.
  • Coordinates referrals and orients new families to the Wraparound process.
  • Provides training and coaching in the wraparound process and EBP.
  • Provides clinical supervision to interns or field-placements if requested by agency
  • Provides mental health services to complex youth and families if needed to assist with language or coverage needs.
  • Assesses and oversees 24/7 on-call process and service for families served, participates in rotation and makes improvements as needed.
  • Provides monthly reports, project plans, annual goals, and oversees tracking systems that provide data to the county and program.

QUALIFICATIONS

  • Education, Training, and Experience.
    • Masters Degree in Social Work, Marriage and Family Counseling, Psychology or related field. California licensure as LCSW, MFT, Ph.D.
    • Three years of clinical experience working with high-needs youth and families.
    • Two years of clinical supervisory experience.
    • Minimum of 1 year experience managing a program required (program development experience preferred, also).
    • Bi-lingual and/or bi-cultural knowledge and experience preferred.
  • Experience working in mental health setting emphasizing a family systems perspective .
  • Skills in crisis intervention, suicide assessment, conflict resolution, and interpersonal communication.
  • Experience working with schools, County Probation, Department of Social Services and Behavioral Health, and other larger county systems.

Clinician

REPORTS TO: CLINICAL PROGRAM MANAGER


OVERVIEW: The Clinician is responsible for providing direct clinical services that include assessment, treatment planning, behavioral interventions, family therapy, group therapy and documentation services in compliance with State and County requirements. This employee reports to the Clinic Program Manager and provides team support and community liaison functions. Client caseload will primarily be community-based and reflect the agency client population serving the needs of high risk youth, adults, and families.


RESPONSIBILITIES INCLUDE (but not limited to):

  • Under the direction of the CPM, works to develop, expand, and maintain outpatient clinical services in a variety of service venues in Santa Clara County.
  • Gather assessment information, orients new families/clients, and writes reports and service plans in a timely manner in compliance with state and county Medi-cal guidelines.
  • Engage family members, allies, and stakeholder in a collaborative process of family driven services and collaborations as directed in the Transformational Care Planning model.
  • Provide direct psychotherapeutic services including individual, group, and family therapy focused upon behavioral outcomes and skill acquisition that promotes independence and permanency.
  • Liaison with county partners and other stakeholders by providing timely information case management and consultation associated with the assigned caseload.

QUALIFICATIONS:

  • Must have a M.A. in Social Work, Psychology, Marriage and Family Psychotherapy or similar field clinically oriented mental health degree.
  • Must be licensed or licensed eligible to practice in California as an LCSW, MFT or Licensed Clinical Psychologist.
  • Must be familiar with helping children/youth with mental health adjustment, symptom resolution and behavioral problems, with experience in California preferred.
  • Must have a demonstrated post-degree track record of at least 2 years success with high need and multi-problem families and youth.
  • At least 1 year experience of cultural competence with high-need Latino client population and/or bi-cultural education delivery experience.


Care Manager

OVERVIEW: The CM provides direct support, case management, and utilization review services to assist emotionally disturbed youth/young adults and their families with meeting their unique individualized goals while connecting them with resources in the community, from entry to exit.


QUALIFICATIONS:

  • Bachelor’s Degree in Social Work (preferred) and/or two years full-time equivalent experience in the mental health field preferred
  • Experience working in a community-based, human services, school-based, or mental health setting preferred 
  • Experience engaging with Transition Aged Youth (TAY) required
  • Excellent interpersonal skills with demonstrated leadership in facilitating and linking services among a diverse variety of mental health programs.
  • Demonstrated time management skills
  • Proven skill in Medi-Cal documentation standards and verbal communication skills
  • Understanding of transformational care planning & targeted case management principles of service delivery.
  • Possesses the ability to motivate and maintain effective working relationships with staff and partners

  • Must be able to work in a Community Care Licensed Facility

RESPONSIBILITIES INCLUDE (but not limited to):

  • Serves as the key coordinator for all UCG services to assigned clients by specified population and/or program site, and makes recommendations as to best integration of services.
  • Creates individualized care plan for each client covering all life domains.
  • Participates in weekly Utilization Review / Treatment Planning meetings.
  • May participate in providing input into the intake process of potential/new residents.
  • Utilizes a variety of engagement and monitoring approaches to establish and maintain a relationship with youth throughout their journey in UCG.
  • Manages a caseload of up to 18 youth.  Maintains individual contact with each youth at least weekly to discuss goals and track progress towards goals during the initial phases of treatment planning and services delivery, to no less than one time per month after stabilization. 
  • Works with youth to understand the purpose for any medications that they are prescribed, helping them learn how to refill prescriptions as needed.  Ensures youth does not run out of medications.
  • Accompany youth, as appropriate, to psychiatrist appointments (as applicable).
  • Maintain ongoing contact with outside persons/entities such as: natural supports, family, referring agency/case carrying agency, ILP, other and community resources. Participate in relevant team meetings such as: Family Support Team meetings, TDMs, and emancipation conferences. Serve as a representative of UCG and liaison with these parties.
  • Support the goals outlined in the Transition Independent Living Plan and work with youth to identify additional goals and tasks as needed. 
  • Provide on-call back-up on assigned weekends or as necessary during crisis, and regular communication to treatment team as to any changes or updates to the individualized care plan.
  • Responsible for completing reports, notes and Medi-Cal paperwork in a timely manner. Responsible for inputting data into the client database accordingly.
  • Works with Compliance Officer, Finance department, and Clinical Program Managers to properly manage the utilization of services resources in as efficient a manner as possible.
  • Facilitate skill building groups for youth.
  • Participate in house meetings, staff meetings as appropriate, and County THPP meetings.
  • Attend required trainings, and meetings as directed.
  •  Collaborate with other team members, such as the Clinician, Facility Manager and Residential Counselors to better serve the youth.
  • Negotiates any transition, crisis or change in the school program.
  • Provide information and referral for youth to connect them with the community and assist/prepare them for transition to adulthood.
  • Provide transportation as needed when it fits within the resident’s goals.
  • Complete duties as outlined in the job duties matrix.


TBS Clinician

Location: San Jose

IMMEDIATE SUPERVISOR: TBS Program Supervisor
DEPARTMENT:  Mental Health
FLSA:  Exempt, Full-Time


OVERVIEW: The TBS Clinician, under the supervision of the TBS Program Supervisor, and in collaboration with the primary mental health provider, coordinates the development and implementation of TBS Service Plans. The Clinician provides both direct and indirect services to children and their families in order to maximize the involvement of all persons in the implementation of the TBS Service Plan and to maintain the child at the lowest appropriate level of care. The Clinician provides 1-1 behavioral services to youth and their families in the community. This position requires the ability to utilize independent judgment in the application of the service plan; and facilitates the integration and coordination of services with internal Unity Care programs and external agencies.


QUALIFICATIONS:

  • MA in Psychology, social work or related field
  • Registered with the Board of Behavioral science as MFTi or ASW, or licensed by the BBS as a MFT or MSW. 
  • Two or more years of direct practice experience working with emotionally disturbed children, adolescents, and families.
  • Demonstrated experience in the development of Behavioral Plans through the use of the Functional Assessment process preferred. Understanding of behavioral principles and procedures.
  • Experience in engaging parents as partners and working successfully with families from diverse ethnic and cultural groups.
  • Ability to effectively facilitate meetings and motivate others to work collaboratively.
  • Strong organizational and planning skills.
  • Ability to work effectively both independently and as member of a multidisciplinary team.
  • Excellent interpersonal, facilitation, and service management skills.
  • Experience in crisis intervention and conflict resolution is preferred. 
  • CA driver’s license, good DMV report; ability to be insured by agency insurance carrier.

  • Able to pass criminal clearance as required by Title XXII licensing regulations.

RESPONSIBILITIES INCLUDE (but not limited to):

  • Serves as a role model and advocate for children and families involved in TBS.
  • Maintains professional standards regarding youth/family rights, HIPAA confidentiality requirements, and clinical responsibilities such as duty-to-warn.
  • Monitors and ensures effective linkages with community and agency service providers.
  • Establishes and maintains positive and effective working relationships with internal staff and external resources.
  • Assumes responsibility for the TBS Service Plan management activities and implementation of services/coordination of resources for children and families within Santa Clara County guidelines and timelines.
  • Develop the Functional Behavior Assessment and development, implementation, and evaluation of individualized TBS Service Plans. Activities to include behavioral observation utilizing various methods and strategies for purposes of assessment and intervention planning.
  • Provides 1-1 specialized behavioral interventions to assist youth to achieve more independent level of functioning in the least restrictive environment.
  • Identifies target behaviors for use in development of behavioral goals with measurable outcomes.
  • Develops/implements incentive procedures and other pertinent interventions to assist child/family in attaining behavioral goals.
  • Assumes responsibility for configuring and facilitating TBS Family Support Team when one is not already in place.
  • Participates in/facilitates monthly Family Support Team meetings for development and ongoing evaluation of individualized TBS plans.
  • Monitors and facilitates the provision of strength-based support by constantly reevaluating and restructuring individual plans as needed, ensuring that families have access to resources, and maintaining effective relationships with appropriate individuals and resources, such as social workers, teachers, and probation officers.
  • Assumes responsibility for transition plan development and implementation.
  • Conducts requisite documentation (i.e., billing and charting) in an accurate and timely manner. Assists in development & refinement of existing documentation and data collection systems.
  • Participates in weekly team meeting and clinical supervision. Attends training sessions or workshops as needed to meet agency or state guidelines.

TBS Coach

OVERVIEW: The TBS Coach is responsible for the care and quality of one to one behavioral coaching services. The TBS Coach insures all services are in compliance with State and County requirements for TBS. Works flexible hours that may include evenings and weekends as necessary, depending on youth and family needs and availability.


QUALIFICATIONS:

  • Must have at least 2 years experience as a mental health worker or special education assistant/classroom aide
  • Must have experience with the problems and needs of children and youth with severe behavioral disturbance and mental illness
  • Must be familiar with TBS
  • Must have experience with families and children, preferred foster youth and juvenile probation youth and families
  • Must have expertise in crisis intervention services and other short term behavioral skill building activities
  • Must be able to successfully obtain a criminal record clearance as required by Title XXII Licensing regulations
  • Must have a valid California Drivers License
  • Must have a clean DMV record
  • Must meet the State of California minimum insurance and licensing requirements as dictated by the State and Unity Care’s insurance carrier/provider

  • The TBS Behavior Counselor will coordinate with community partners to create a seamless and well-integrated service delivery, in order to provide excellent services to clients.
  • The TBS Behavior Counselor must be able and willing to travel in order to oversee services in multiple counties.

OVERVIEW: Provides vision, goal, and overall leadership to Wraparound process for children and families.  The Wraparound Facilitator I is responsible for developing and coordinating family-centered teams to meet the individualized needs of children and families. This position is responsible for the development of an integrated plan and leadership to ensure implementation of services and supports to the child and their family/caregiver that is responsive to concerns and builds on strengths. The Wraparound Facilitator I provides facilitation, assessment, and case management services including all related documentation for planned and provided services. This position works independently in the family’s home and a variety of community and/or treatment settings. This position works directly with special needs youth and their families in a manner consistent with Wraparound Values and program practices. 


QUALIFICATIONS:

  • Bachelor’s degree and 5 years experience working with special needs children and their families
  • Specialized knowledge and/or abilities in positive behavioral management, recreational, art therapy preferred.
  • Good verbal & written skills.

RESPONSIBILITIES INCLUDE (but not limited to):

  • Supports the provision of Wraparound Services in a manner that is highly individualized, family-focused, team driven, strength-based and directed by the needs of the child and family. Provides an advanced level of facilitation and coordination in the development of individualized services for children and families.
  • Works with families and children from a strength-based perspective. Assesses customers’ concerns, needs and expectations (child, family, referral sources, and community). Participates with families as a complete unit with focus on individual strengths/concerns/needs. Facilitates the development and refinement of effective reactive and proactive safety plans. Develops initial reactive safety plan within first 7 days of contact with the family.
  • Provides meeting minutes to all Child and Family Team members to support communication, inclusion and accountability. Minutes reflect decisions, agreements, and outcomes of prior strategies.
  • Provides quality, culturally proficient and relevant services to children and their families in community-based and in-home settings.  Promotes cultural awareness and competence within the Program and the agency.
  • Supports the ongoing development of a strength-based system philosophy built upon Wraparound Values and Principles within the service components of the Wraparound Program. Demonstrates and promotes Wraparound Values and Best Practices.
  • Works in collaborative partnership with all team members promoting a participative team approach characterized by mutual accountability. Participates effectively in multidisciplinary clinical teams, internally and externally.
  • Researches and utilizes community resources to meet family needs. Supports Family Specialists in the development of a multitude of resources within the community to serve children and families.
  • Documents work in a manner consistent with all relevant laws and regulations, including MediCal documentation standards.
  • Participates in 24 hour on-call system.
  • Contact families within 24 hours of receiving referral, files safety plans, and orient each CFT to Wraparound at first meeting.
  • Facilitate first CFT meeting, complete and file consents for treatment, release of information, and other required documentation within one month of intake.
  • Encourage as many natural support persons on each CFT as possible.
  • Develop and present IFP’s to the Community Team within two months of intake. Under appropriate supervision and consistent with program guidelines; organizes and presents clinical evaluation and initial service plan to staff and community partners.
  • Submit all necessary documentation in a timely manner for placement in child’s binder.
  • Develop IFP’s based on child/family strengths and comprehensive needs assessments. Oversee the direct implementation of the IFP & frequently perform in a case management capacity. Monitor IFP’s continually for implementation and needed modifications.
  • Track use of Family Specialist hours and Flexible Funds. The expectation is that both will decline eventually. Understands and implements funding policies/procedures and trains Child and Family Teams in stewardship practices.
  • Transition from categorical services to natural, community based resources over time and resource opportunities for children in their own community.
  • Bill within 24 hour and for a total of between 60-80 hours per month.
  • Complete discharge paperwork as soon as the inactive status is agreed upon in CFT and/or with referring agency.
  • Contact referring social worker/mental health/probation officer within 24 hours of intake.
  • Provides a leadership role in linking parents, children and staff when a crisis arises.
  • Attend all mandatory meetings and trainings.
  • File weekly schedule each Friday. Manages flexible schedule consistent with family and program needs.
  • Maintain excellent Time-Management and prioritization skills.

QUALITIES OF FACILITATOR I:

  • Proficient and timely in the proper documentation of direct services consistent with MediCal regulations and Agency/Program Policies and Procedures. Documents necessary information as required within the program, including but not limited to client demographics and progress documentation, report writing and accurate data collection to assess progress and outcome
  • Participates in internal audits.
  • Attends all mandatory meetings and trainings.
  • Maintains excellent time-management skills. Effective personal organizational systems for ensuring paperwork/information management (filing, updating community resources, flex fund paperwork management) are developed and maintained to support timelines and priorities.

Family Specialist I and II (3 openings)

OVERVIEW:

The Family Specialist II is responsible for providing guidance, services and supports at an advanced level to children and their families/caregivers in a variety of environments, including the home, school and community. This position primarily works directly and intensively one-on-one with children and youth, within the context of their setting. The specialist provides behavioral interventions, skills training and community resourcing and utilizes a strengths-based approach. The program primarily serves African American youth and their families/caregivers. Flexible hours are required that may include evenings and/or weekends, depending on the youth's needs and availability. Local Candidates Only.


MINIMUM QUALIFICATIONS:
  • B.A. in Social Work, Psychology, or other related social services field, and/or minimum of 2-5 years experience working with special needs youth. Experience may substitute for a degree. MHRS County Certification preferred.
  • Must have experience and be familiar with effective interventions in working with youth in the juvenile justice probation system, with youth who are involved with gangs or who are gang-entrenched, and with youth with Substance abuse problems.
  • Wraparound experience preferred.
  • Specialized knowledge and/or abilities in positive behavioral management preferred.
  • Experience with Foster Care and Mental Health are a plus.
  • Experience with Medi-Cal billing policies and procedures, and processing Medi-Cal billing in a timely manner preferred.
  • Availability to participate in 24-hour on-call program rotation.
  • Speak, read, and/or write another language fluently, with a demonstrated ability to work with diverse ethnic populations a plus.
  • Must have valid CA Drivers License, insurance, and reliable transportation.
  • Must be able to meet and receive a criminal records clearance as required by Title XXII licensing requirements.

Resident Property Manager

OVERVIEW OF ROLE: The Resident Property Manager is responsible for the day-to-day oversight of the apartment complex. This person will be responsible for public relations and will oversee all work associated with the property maintenance, security, and safety activities of the complex. This position requires residency on the property. Lodging (including utilities) is offered instead of salary.


QUALIFICATIONS:

  • Knowledge of principles and practices of property management and applicable federal, state, and local laws and regulations

Residential Counselor (PART-TIME, FULL-TIME, ON-CALL)

RESPONSIBILITIES:
Unity Care is currently seeking qualified candidates with excellent judgment, background and experience with at-risk adolescents with gang diversion/intervention and substance abuse issues in our San Jose level 12 youth homes. Under the direction of the Facility Manager, Level 12 Counselors interact with residents in all aspects of their daily routines, help them meet treatment goals, and assist in managing their behavior. They also observe behavior, interactions and individual needs of adolescents within the house, reporting such observations to appropriate staff as well as providing documentation. They participate in primary crisis prevention, and provide one-on-one counseling as directed by the Facility Manager, all the time protecting the dignity, self-respect, rights, and individuality of each adolescent in care. They will assist in planning and using recreation as a form of therapy, and dispense and log medication as directed, always providing for their physical safety, and assure safety procedures are followed.


REQUIREMENTS:

  • Must be at least 21 yrs. of age, with a minimum of 2+ yrs. experience working with at-risk youths or adolescent children in a residential setting preferred
  • At least 60 completed college units from an accredited institution preferred
  • Demonstrated skills & training in crisis interventions, suicidal assessment, conflict resolution and interpersonal communication skills
  • California Drivers License, and Reliable Transportation
  • Must clear a criminal background, DMV, and health screen check
