Provides a wide range of mental health services for youth in the Child Welfare, Juvenile Justice and School Based systems who are currently receiving other Unity Care services. Services are also provided for the families and caregivers of the youth.
Provides one-on-one mental health services for children with severe behavioral and emotional issues so they can successfully transition from a high level of care to a less restrictive setting or stay in their current setting.
Establishes Family Teams to increase the capacity of families to meet their children's needs so that emotionally and behaviorally challenged youth can stay with their family and community in the most normative and least restrictive environment.
Helps and strengthens African American families in crisis by advocating for them, providing parent education, connecting them with African American and other community resources, and providing counseling.
Assists families who are currently in the family reunification process to fulfill their court-ordered case plans, locate appropriate community resources, navigate the child welfare system, and increase positive communication and parenting skills.
Helps stabilize and maintain children in their foster or adoptive home by providing in-home visits, advocating for the children/families, facilitating communication with county agencies and health services, and connecting families with community resources.
Provides facilitation of meetings with the family and other people the family identifies as resources to them, in order to develop a safety plan so that parents can reunify with their children or keep their children at home or with relatives.
